2024/2025 Business License Forms

If you are completing your annual business license renewal, you will need to complete ALL 3 OF THE FOLLOWING FORMS:

1. Business License Renewal Application
2. Business Emergency Contact Sheet
3. Alarm Registration Form (if you have a security alarm)

In addition to the above, if you hold any of the following licenses, you must complete these renewal applications in addition to the above:

• Liquor License Renewal Application
• Video Gaming Application
• Business Emergency Contact Sheet
• Alarm Registration Form (if you have a security alarm)
• Vending Application (if applicable)

 Applications required for new businesses:

• New Business Application
• New Liquor License Application (if applicable)
• New Video Gaming Application (if applicable)
• Cannabis Dispensary Retail Sales License Application
• Business Emergency Contact Sheet (all businesses)
• Alarm Registration Form (if you have a security alarm)
• Vending Application (if applicable)
